Can you pay for GLP-1 medication with an HSA or FSA?

Under IRS rules a prescribed GLP-1 used to treat a disease a physician diagnosed, such as obesity, is a medical expense, which is what HSA money pays for; an FSA pays what its own plan specifies. Of the 76 sellers GLPcompare tracks, 36 say on their own pages that they take HSA or FSA payment, 14 say their plans are HSA or FSA eligible, 3 describe paying by card and claiming the receipt and 2 say they do not take it. Each statement below is quoted from the seller's page with the day it was read.

The rule, in the IRS's words

What the IRS says

Quoted from the IRS's own publications, last updated by the IRS on Apr 30, 2026. This is the general rule, not advice for your own taxes or plan.

Prescribed medicines
“You can include in medical expenses amounts you pay for prescribed medicines and drugs. A prescribed drug is one that requires a prescription by a doctor for its use by an individual.”

IRS Publication 502 (2025), Medical and Dental Expenses

Weight loss
“You can include in medical expenses amounts you pay to lose weight if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease). This includes fees you pay for membership in a weight reduction group as well as fees for attendance at periodic meetings.”

IRS Publication 502 (2025), Medical and Dental Expenses

What an HSA pays for
“In general, "qualified medical expenses" means amounts paid by the HSA beneficiary for "medical care" (as defined in Code section 213(d)) for the individual, the spouse of the individual, and any dependent of the individual, but only to the extent the amounts are not compensated for by insurance or otherwise. A distribution for anything else will be subject to income tax and may be subject to an additional 20% tax.”

IRS Publication 969 (2025), Health Savings Accounts and Other Tax-Favored Health Plans

What an FSA pays for
“Qualified medical expenses are those specified in the plan that would generally qualify for the medical and dental expenses deduction.”

IRS Publication 969 (2025), Health Savings Accounts and Other Tax-Favored Health Plans

Publication 502 also excludes a drug ordered and shipped from another country unless it was imported legally. Compounded semaglutide and tirzepatide are dispensed on a prescription too; the rule turns on the prescription, not on FDA approval.

Questions

GLP-1 and HSA or FSA: common questions

Is GLP-1 medication HSA or FSA eligible?

A GLP-1 medication such as Wegovy, Zepbound, Ozempic, Mounjaro, the Wegovy pill or Foundayo is dispensed only on a prescription. IRS Publication 502 counts prescribed medicines as medical expenses ("You can include in medical expenses amounts you pay for prescribed medicines and drugs. A prescribed drug is one that requires a prescription by a doctor for its use by an individual.") and weight loss as one when it treats a disease a physician diagnosed, such as obesity. HSA money pays for medical care as that rule defines it, and an FSA pays what its own plan specifies, so check your plan. Publication 502 also says: "You can include in medical expenses amounts you pay to lose weight if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease). This includes fees you pay for membership in a weight reduction group as well as fees for attendance at periodic meetings." Money taken from an HSA for anything that is not a qualified medical expense will be subject to income tax and may be subject to an additional 20% tax (IRS Publication 969). Can I pay a telehealth membership or visit fee with my HSA?

Publication 502 says medical expenses include "payments for legal medical services rendered by physicians, surgeons, dentists, and other medical practitioners", and that weight loss treating a diagnosed disease includes "fees you pay for membership in a weight reduction group". Whether a seller's membership counts on your claim, and whether your FSA covers it, depends on what the fee pays for and on your plan; several sellers below say what their HSA or FSA payment covers.
