Microdose plans are compounded, off-label and untested in published trials.

Compounded drugs are not FDA-approved: the FDA does not review them for safety, effectiveness or quality before they are marketed. FDA: concerns with unapproved GLP-1 drugs used for weight loss

Wegovy's label starts the injection at 0.25 mg once weekly for 4 weeks, then raises the dose every 4 weeks; the maintenance dosage for weight reduction in adults is 1.7 mg or 2.4 mg (recommended) once weekly. Wegovy prescribing information (DailyMed, June 30, 2026)

Zepbound's label says the 2.5 mg dosage is for treatment initiation and is not approved as a maintenance dosage; the recommended maintenance dosage for weight reduction is 5 mg, 10 mg or 15 mg once weekly. Zepbound prescribing information (DailyMed, September 2, 2026)

ClinicalTrials.gov lists 3 registered studies of GLP-1 microdosing, none with posted results (checked Oct 19, 2026): AgelessRx's randomized, placebo-controlled early phase 1 trial of microdosed GLP-1 for health, quality of life and longevity measures, planned for 150 people, enrolling by invitation NCT07092605; a University of Texas Health Science Center at Houston phase 2 trial of semaglutide 0.5 mg weekly after a 2 mg course, against stopping, planned for 30 people with HIV, recruiting NCT07325500; Noom's observational cohort (FLOURISH) comparing microdose and standard-dose compounded semaglutide and tirzepatide over 24 months, not yet recruiting NCT07588984.

Read prices in context. Compare the same product, dose, and payment terms. A prepaid monthly rate is not a monthly payment. Dose changes, introductory terms and added fees affect what you pay.

Missing prices are not estimated. A listed maintenance dose is not a dosing recommendation.

The FDA declared the tirzepatide (December 2024) and semaglutide (February 2025) shortages resolved, ending the exemption that let pharmacies mass-produce compounded copies, and the 503A/503B enforcement-discretion grace periods ended in 2025. The FDA is now actively enforcing (warning letters and takedowns, including a September 2026 warning letter to Empower Pharmacy, a large compounder, over products it called essentially copies of approved semaglutide and tirzepatide), and in 2026 proposed permanently excluding semaglutide, tirzepatide, and liraglutide from the 503B bulks list (the public comment period, extended 30 days, closed July 30, 2026; a final decision is pending). Liraglutide is different: it is still on the FDA drug shortage list (brand Victoza and Saxenda in limited supply, generics available, as of September 2026). Compounded GLP-1s are not FDA-approved and are now generally limited to narrow, patient-specific 503A prescriptions, so broad low-cost availability has largely wound down. They also carry safety risks the branded products do not: FDA adverse-event reports, at least one sterility recall (ProRx, October 2025), and counterfeit product found in the US supply chain, so a low price is not a low-risk signal. Prices and availability below may lag these changes; confirm current terms with the provider.

Every plan is compounded, and insurance does not cover compounded GLP-1s; these are the sellers' own advertised prices, and the conditions listed with each apply.

What is the cheapest microdose GLP-1?

Ranked per 30 days, the lowest is microdose semaglutide at NiceRx at ≈ $138 per 30 days (advertised as $129/28 days), then microdose tirzepatide at Enhance.MD (≈ $181 per 30 days) and microdose tirzepatide at NiceRx (≈ $181 per 30 days). The lowest advertised figure, $79/mo for microdose semaglutide at CoreAge Rx (a starting price), has no verified supply period, so it is not ranked per 30 days. Microdose plans are compounded and not FDA-approved, so a lower price says nothing about quality.

How much does microdose semaglutide cost?

11 sellers publish a microdose semaglutide price, from $79/mo for microdose semaglutide at CoreAge Rx (a starting price) to $249/mo for microdose semaglutide tablets at Fridays. The lowest that states its supply is ≈ $138 per 30 days for microdose semaglutide at NiceRx. Wegovy's label starts the injection at 0.25 mg once weekly for 4 weeks, then raises the dose every 4 weeks; the maintenance dosage for weight reduction in adults is 1.7 mg or 2.4 mg (recommended) once weekly. Regular compounded semaglutide prices

How much does microdose tirzepatide cost?

12 sellers publish a microdose tirzepatide price, from $116/mo for microdose tirzepatide at Telos Rx (a starting price) to $274.99/mo for microdose tirzepatide at Ivim Health. The lowest that states its supply is ≈ $181 per 30 days for microdose tirzepatide at Enhance.MD (12-month plan paid upfront) and NiceRx. Zepbound's label says the 2.5 mg dosage is for treatment initiation and is not approved as a maintenance dosage; the recommended maintenance dosage for weight reduction is 5 mg, 10 mg or 15 mg once weekly. Regular compounded tirzepatide prices

Does microdosing GLP-1 cost less than a regular plan?

Only the same seller's two plans can be compared, and only when both state their supply. 3 plans allow that: Enhance.MD's microdose tirzepatide is $119 less per 30 days than its regular plan; NiceRx's microdose semaglutide is $21 less per 30 days than its regular plan; NiceRx's microdose tirzepatide is $86 less per 30 days than its regular plan. At CoreAge Rx, the microdose plan is advertised at the same price as the regular plan.

Is GLP-1 microdosing FDA-approved?

No. Every microdose plan tracked here is compounded, and compounded drugs are not FDA-approved: the FDA does not review them for safety, effectiveness or quality before they are marketed. Wegovy's label starts the injection at 0.25 mg once weekly for 4 weeks, then raises the dose every 4 weeks; the maintenance dosage for weight reduction in adults is 1.7 mg or 2.4 mg (recommended) once weekly. Zepbound's label says the 2.5 mg dosage is for treatment initiation and is not approved as a maintenance dosage; the recommended maintenance dosage for weight reduction is 5 mg, 10 mg or 15 mg once weekly. A microdose is a dose below those maintenance doses, chosen by the prescriber. FDA: concerns with unapproved GLP-1 drugs used for weight loss

Has GLP-1 microdosing been tested in clinical trials?

Not with published results. ClinicalTrials.gov lists 3 registered studies of GLP-1 microdosing, none with posted results (checked Oct 19, 2026): AgelessRx's randomized, placebo-controlled early phase 1 trial of microdosed GLP-1 for health, quality of life and longevity measures, planned for 150 people (NCT07092605, enrolling by invitation); a University of Texas Health Science Center at Houston phase 2 trial of semaglutide 0.5 mg weekly after a 2 mg course, against stopping, planned for 30 people with HIV (NCT07325500, recruiting); Noom's observational cohort (FLOURISH) comparing microdose and standard-dose compounded semaglutide and tirzepatide over 24 months (NCT07588984, not yet recruiting). The weight-loss results on the FDA labels come from trials of the full maintenance doses. ClinicalTrials.gov: NCT07092605
