From each FDA label

Do GLP-1s affect birth control?

The labels differ. Zepbound (4 weeks), Mounjaro (4 weeks) and Foundayo (30 days) tell people on the pill to switch to a non-oral method or add a barrier method for that long after starting and after each dose increase. Non-oral methods should not be affected. Wegovy and Rybelsus/Ozempic tablets report no clinically significant differences in the pill's hormones when taken with semaglutide.

Rybelsus and Ozempic tablets

  • No clinically significant differences in the pharmacokinetics of the following drugs were observed when used concomitantly with semaglutide: lisinopril, S-warfarin, R-warfarin, metformin, digoxin, ethinyl estradiol, levonorgestrel, furosemide or rosuvastatin.

Zepbound

  • Advise patients using oral hormonal contraceptives to switch to a non-oral contraceptive method, or add a barrier method of contraception for 4 weeks after initiation with Zepbound and for 4 weeks after each dose escalation.
  • Hormonal contraceptives that are not administered orally should not be affected.
  • Use of Zepbound may reduce the efficacy of oral hormonal contraceptives due to delayed gastric emptying.
  • Following administration of a combined oral contraceptive (0.035 mg ethinyl estradiol and 0.25 mg norgestimate) in the presence of a single dose of tirzepatide 5 mg, mean C max of ethinyl estradiol, norgestimate, and norelgestromin was reduced by 59%, 66%, and 55%, while mean AUC was reduced by 20%, 21%, and 23%, respectively.

Mounjaro

  • Advise patients using oral hormonal contraceptives to switch to a non-oral contraceptive method or add a barrier method of contraception for 4 weeks after initiation and for 4 weeks after each dose escalation with Mounjaro.
  • Hormonal contraceptives that are not administered orally should not be affected.
  • Use of Mounjaro may reduce the efficacy of oral hormonal contraceptives due to delayed gastric emptying.
  • Following administration of a combined oral contraceptive (0.035 mg ethinyl estradiol and 0.25 mg norgestimate) in the presence of a single dose of tirzepatide 5 mg, mean C max of ethinyl estradiol, norgestimate, and norelgestromin was reduced by 59%,66%, and 55%, while mean AUC was reduced by 20%, 21%, and 23%, respectively.

Foundayo

  • Advise patients using oral hormonal contraceptives to switch to a non-oral contraceptive method or add a barrier method of contraception for 30 days after initiation with Foundayo and for 30 days after each dose escalation.
  • Hormonal contraceptives that are not administered orally should not be affected.
  • Advise women of childbearing potential to use effective contraception during treatment with Foundayo.
  • Because delayed gastric emptying may affect the absorption of oral medications, advise patients using oral hormonal contraceptives to switch to a non-oral contraceptive method, or add a barrier method of contraception for 30 days after initiation with Foundayo and for 30 days after each dose escalation.

Saxenda

  • A single dose of an oral contraceptive combination product containing 0.03 mg ethinylestradiol and 0.15 mg levonorgestrel was administered under fed conditions and 7 hours after the dose of liraglutide at steady state.
  • Liraglutide lowered ethinylestradiol and levonorgestrel C max by 12% and 13%, respectively. There was no effect of liraglutide on the overall exposure (AUC) of ethinylestradiol.
  • Liraglutide delayed T max for both ethinylestradiol and levonorgestrel by 1.5 h.

Victoza

  • A single dose of an oral contraceptive combination product containing 0.03 mg ethinylestradiol and 0.15 mg levonorgestrel was administered under fed conditions and 7 hours after the dose of Victoza at steady state.
  • Victoza lowered ethinylestradiol and levonorgestrel C max by 12% and 13%, respectively. There was no effect of Victoza on the overall exposure (AUC) of ethinylestradiol.
  • Victoza delayed T max for both ethinylestradiol and levonorgestrel by 1.5 h.

Byetta

  • For oral medications that are dependent on threshold concentrations for efficacy, such as contraceptives and antibiotics, patients should be advised to take those drugs at least 1 hour before Byetta injection.
  • Repeated daily doses of the oral contraceptive (OC) given 30 minutes after Byetta administration decreased the C max of ethinyl estradiol and levonorgestrel by 45% and 27%, respectively, and delayed the T max of ethinyl estradiol and levonorgestrel by 3.0 hours and 3.5 hours, respectively, as compared to the oral contraceptive administered alone.
  • Administration of repeated daily doses of the OC one hour prior to Byetta administration decreased the mean C max of ethinyl estradiol by 15% but the mean C max of levonorgestrel was not significantly changed as compared to when the OC was given alone.
  • However, the mean trough concentration of ethinyl estradiol was increased by 20% when the OC was administered 30 minutes after Byetta administration injection as compared to when the OC was given alone.
